Incident/Complaint Form

You will be asked:

• Name
• Date/Time of incident
• Witnesses
• Summary of the incident/complaint
• Supporting Documents or Photos

During the investigation, we will ask for written statements from parties involved, taking into account the age of the individuals involved. Some interviews will be necessary. The complaint will be reviewed in its entirety by the Executive Committee, unless there is a conflict of interest. At that point, the involved EC member will recuse themselves.
